New Patient Information
We are glad that you have chosen Oak Ridge Neurosurgery and Spine Center. We hope that you find this website informative and helpful as you prepare for your visit.
- Every effort is made to see you as close to your scheduled appointment time as possible. In order to help us meet this standard, we ask that you arrive with all your MRI and X-ray films. All your current medications listed with the milligram and how many times a day you take the medication. All forms that were mailed to you completely filled out along with a copy of your insurance card. This will allow us to register all of your pertinent information into our computer system.
- Because it is so important that we always maintain accurate information about you, we confirm specific information such as date of birth, telephone, address and insurance information at each visit. While we know that many patients consider this excessive, we find it crucial to have the correct telephone number in order to contact you concerning your health status.
- Please remember to bring your insurance card with you. For your convenience, we do file most insurance plans, but it is extremely important that we have all of the correct information.
- Co-payments, as well as prior balances are collected prior to your visit. This should help to minimize the wait in checking out after you have been seen.
- If you account is ever turned over to a collection agency, you will automatically be released from our practice and your primary care physician will need to find you a new neurosurgeon.
- We've included a copy of our patient information brochure that we hope helps you to get acquainted to some of our policies.
